* Complete the on-line roster         
* Mail the Following 3 items to:
               Tournament Registrar - Milliken Fog Classic
               1263 Park Pacifica Ave.
               Pacifica, CA  94044

    1. Referee Information Form (signed by your Regional Referee Administrator)    
    2 . Official Roster from AYSO system (signed by your Regional Commissioner) 
    3 . A Regional Check made out to "AYSO Region 157" for:

                               U-10                $700*            
                               U-12                $800*            
                               U-14/16/19     $850*  

                                *Includes refundable referee deposit of $150

FOR COACHES:  your "APPLICATION DATE" 
will be the day we receive your COMPLETED application materials, NOT the date you fill out your on-line registration.
